Step 1: Personal Information

Fill in the required personal information accurately in the first section of the form. The following fields are mandatory:

  1. First Name, Middle Name (if applicable), and Last Name
  2. Email Address (Ensure this is an active email you have access to)
  3. Date of Birth
  4. Gender
  5. Contact Number

Note: Make sure to double-check all entries in this section as errors might affect your application.

Step 2: Residence Information

In the second part of the form, you need to enter your residential details. Ensure to provide:

  1. Your current address
  2. Province and district information
  3. Contact information for your home/residence

Step 3: Parent/Guardian Information

Enter the contact details of your parents or guardian. This section includes:

  1. Father's/Mother's Name and Contact Information
  2. Guardian's Information (if applicable)

Step 4: Educational Information

In the educational information section, you will provide details of your previous academic qualifications:

  1. Select your highest degree (Secondary, Bachelor, or other)
  2. Institution Name, Year of Graduation, and Percentage/Grade
  3. You can add multiple educational qualifications by clicking the "Add More Education" button.

Step 5: Payment Information

You will need to upload a scanned copy or image of the payment voucher to complete your application if you want to pay online or you can choose I will pay at college if you want to pay later.

  1. Ensure the payment is made to the correct account before uploading the voucher.
  2. The voucher should be clearly readable.
  3. Fee for Application fee:
    Rs. 125 for Class 11.
    Rs. 1000 for Bachelor Level.
    Rs. 2500 for Masters Level.

Note: Without the payment voucher, your application will not be processed. if you choose I will pay at college you have to pay the application fee at college to get the entrance card.

Step 6: Final Submission

After completing all the sections:

  1. Review all the entered data to ensure there are no mistakes.
  2. Click the "Submit" button to complete your application.

Important Notes

  1. Make sure to fill out all required fields marked with an asterisk (*)
  2. Keep your documents (like payment voucher) ready for upload in the required formats (JPG, JPEG, PNG).
  3. You have to bring your original documents during the entrance examination
  4. If you encounter any technical issues during submission, clear your browser cache or try using a different browser.
